Internship Supervisor Agreement Form – Spring 2021


Internships are planned training experiences that provide an opportunity for students to put academic learning to work. This is an academic course for which
students receive three units of credit. Six units maximum of internship credit is allowed, three of which may be graduate level. Interns are required to complete a minimum of 120 hours with the employer during the academic semester.

Internship placement is contingent upon the consent of a sponsoring faculty member and, where applicable, the passing of a portfolio review.

The sponsoring faculty for the Spring 2021 semester is:

Students have basic skills learned in an academic setting. This may be the student’s first opportunity to practice skills in a professional environment. Internship supervisors are expected to take time to initiate the student into the work environment of their individual company.

We have confidence that supervisors will guide/assist students as needed. We trust that all the company employees will utilize the student’s services in the spirit of the program and expose them to as many facets of the operation as possible.

It is expected that supervisors will notify the sponsoring faculty member of problems as soon as possible.


Students will be treated as employees. They are to be punctual, do work as assigned, ask questions to clarify issues, not expect excessive training, and perform to the best of their abilities at all times.


University Personnel:
University Personnel will match appropriate students with internships. Students will be required to attend a minimum of three meetings with their internship
course faculty during the semester. Students will be required to document their participation through a description of their work. If possible, a portfolio of work should be maintained.

Students must work an equivalent of at least 8 hours per week during the 15-week academic semester. Internship supervisors are under no obligation to continue students past the contract time specified.
